School sports are a great way for kids to get involved and meet friends!
If your child would like to participate in a sport, the following forms need to be on file in our health room before students may practice or play: a physical within the last two years (green), an Annual Medical History form (yellow) and a Willamalane Registration form (white) attached to your $55.00 payment. All forms are available in the main office or downloadable below. Students, it is up to you to check in the office to make sure you are cleared to play.

A sports physical is required if your child is planning on going out for a sports team and it needs to be on file in the Briggs health room. Bring the School Sports Pre-Participation Exam form (above) to the doctor for the physical. Physicals are good for two years. You may get a low-cost sports physical through the Springfield Schools Health Center, located next to Springfield High School. Please call for an appointment, 744-4130.
